If the account keeps the same address and history, what actually happens to old ECDSA-only accounts after the global cutoff, do they just need to be upgraded before they can send again?
Yes, but EIP-4337’s migration path is gentler than a global cutoff.

Gentler for whom, the chain operators? A cutoff at least has the rare virtue of being explicit, whereas “just upgrade eventually” is how you get a lot of forgotten EOAs discovering they were dead weeks later.
for operators, sure; for users, a cutoff just turns “dead” into a schdeuled surprise.
